Following your assessment by a Social Worker/Social Worker Assistant which will have identified your eligible social care needs, a care plan will need to be written. This will explain how these needs are going to be met, who will provide the care you need and the costs involved. This care plan can be completed by your Social Worker/Social Worker Assistant or an Independent Support Broker.
Following authorisation you will need to complete a Direct Payment Agreement. This outlines the budget available to you and the terms and conditions of the direct payment.
